All students who are participating in High School Band (8th through 12th grade) must complete a medical history form each year. All students must also have a physical examination with a doctor or other qualified physician. Any student who completes their physical and medical history paperwork for athletics will not need separate forms for band and vice versa.

This form must be submitted to the band hall office by August 1st in order to be eligible to practice or rehearse for any UIL activity.
